- Activity Information
- Entry for activity title, sponsor, description, agenda and learning resources.
- Contact Information
- Activity Coordinator, Industry Contact, Activity Logistics Contact, and Registration Contact. Contact information for the Activity Coordinator MUST be entered or the application cannot be completed. The confirmation number will be sent to the Activity Coordinator's email address.
- Activity Educational Objectives
- Objectives (5 minimum) and post-meeting use must be completed.
- Faculty
- Faculty (speakers/presenters) will be entered on the final page. Full address and contact information would be appreciated but only name, credentials, title, and company are mandatory.
After the application is completed, an email confirmation will be sent to the email address of the Activity Coordinator, along with the confirmation number. The confirmation number is to be used with any communications with ASCP. To proceed, click the "Make Application" button below.
